I would also add a mini facilitation framework which works for certain meetings not all:
Ask participants:
A. In the beginning: Why are you here?
B. Towards the end: Did you get what you were looking for in this meeting?
C. If not why?
I would find a bit odd that I get invited to a meeting and the organizer asks me "Why are you here". As a meeting organizer, if some attendees have not contributed to the meeting, probably they shouldn't have been there. Saving Changes...

Agreed! Hence mentioning that this is not for all meeting types. But if a meeting/call is for shared goals and all members have already identified the need for a meeting this can actually work out very well. It helps all participants to learn what the other person expects to gain from the meeting. Examples include, conflict resolution, brainstorming, solutions architecture. Saving Changes...

I would like you to consider the differences in cultural that make meeting be completely different depending on the country. There are countries where a meeting is held with a sacred agenda that is followed in serial order by the minute and others where punctuality is almost irrelevant and only one big topic is discussed with no written agenda.
Perhaps a way to select the 'style' of the agenda beforehand and notify all attendees so they know what kind of meeting it is and what is expected before, during and after.
That and a way to connect the agenda with concrete projects or issues.

When I led one of the first team meetings with a new team I share with them the cost of having all us during 30min. I found this HBR calculator very useful.
Since then, they understand how valuable is the time and the importance to keep focus, on time and with the points noted in the agenda. Is a support for Kiron notes :)

From my experience!
Ask the team / meeting participants to bring potential solutions! People tend to come to the meetings without preparation and as such a lot of time is lost discussing the issues and not the solutions!
So unless you want to meet and discuss the issues focus on pre meeting preparation and make sure that all the participants are aware of what is the meeting focus and purpose! Saving Changes...

Adding to all valuable inputs here, i believe that more media interface in meetings can help achieve the meeting objectives, i.e. Presentation of Agenda items / agenda history, Writing Boards and a conclusive document (like minutes ) in the end. Video/Audio recordings of Meetings can be also useful.
