Survey questions needed.

What are your top 5-10 survey questions to ask stakeholders in an anonymous survey immediately after a project has closed to measure a project managers effectiveness including soft skills? Do they differ if Waterfall, Iterative Waterfall or Agile? Any suggestions if DevOps projects?

I can suggest one, and I assume you are giving them a sliding scale question, or just open answers? I would ask "How do you feel the product delivered matches your requirements?"

One I would ask is: "Would you seek out this same team as resources to build your next product?" A yes or no to that question says a lot. But I would also follow with Sante's question which will elicit specific likes and dislikes.

For me that most important thing is to know how the PM's level of communication and engagement for stakeholders was so my question would be:

Were you satsfied with the level of communication and engagement throughout the project ?
(Measured on a level from 1 - 10)

I would thank them for releasing the performance bond and other documents and ask:
1. What are your overall performance evaluations? (scale it from 0-10) & Why
2. Please list your suggestions for future improvement?
