Format: Log

Use the issue log to maintain a consolidated view of issues that have been encountered on the project, and their resolution. Issues represent challenges and problems that need to be solved in order for the project to be successful. Keeping track of issues is important to make sure that they are addressed, and that significant items don’t get ignored or fall from attention.

Issues are related to but separate from risks. While risks represent uncertainties in terms of what might happen, issues reflect problems that are happening.

Capture issues as they occur to keep them in focus. These might get identified in meetings, arise during work sessions or get flagged when updating your project sponsor or steering committee. They may relate to the work of the project, challenges around resourcing, difficulties with a vendor or organizational barriers that need to get addressed.
