• Clarify. This template is for a post-implementation review, not simply a lessons learned exercise. 
• Include the PM. Often when a post-implementation review is conducted, the project manager may be off working on other projects. Invite the PM to be involved in the exercise, as she will have a great deal of valuable information to add to the process.
• Be inclusive.  Make sure you include attendees from a variety of sources including the project team, the customer, the sponsor, and other stakeholders.
