This RAID (Risks, Actions, Issues and Decisions) log is a simple, effective way to track events in a project and create an audit trail. It can be dropped into SharePoint or any project repository, and with just a couple of formatting modifications is ready to use immediately.  

There are four (4) tabs, one for each RAID category. Each sheet has its own unique numbering scheme (RS-001, AS-001, IS-001, DS-001) to easily determine the type of event if the listings are ever consolidated. The project manager can elect to use this feature or simply consolidate all project event entries to the ISSUES Log tab and hide the other tabs.  

The PRIORITY column has a dropdown menu to categorize the severity of the event as (LOW, MEDIUM, HIGH or CRITICAL).  The STATUS column has a dropdown menu to categorize the current status of the entry as (1-Not Started, 2-WIP [work in progress],  3-In Testing, 4-Monitoring, 5-Cancelled, 6-Closed).  Formulas for these dropdowns or any others you wish to incorporate are maintained on the DATA tab.
